Where to get dog collar ?

Is there a dog collar I can get that can have the dogs info visibly in
the collar and not on a tag?

Dog Name
Phone
Rabies info
Chip info.

I heard somewhere (don't recall where) that some animal control officers use
the shape of the rabies tag to determine from a distance if the vaccine is
current or not, and use this to make decisions as to if/how they should
approach the dog... Having that info on the collar might not be all that
helpful.

Also depending on where you live, many areas have municipal license tags
that must be attached to the collar anyways...
